Order of the Ministry of Health and Social Development of Russia dated 02.05.2012 No. 441n approved the Procedure for issuing certificates and medical reports by medical organizations (hereinafter referred to as the Procedure), which entered into force on 17.06.2012. Let's consider in the article the main provisions of this document.

To whom and by whom medical certificates and opinions are issued

Previously, the general procedure for issuing medical certificates and medical reports was not approved by the Russian Ministry of Health and the Russian Ministry of Health and Social Development. At various times, these departments approved and are currently operating separate forms of medical certificates and medical reports, as well as the procedure for their issuance. For example, the Order of the Ministry of Health and Social Development of Russia dated September 28, 2010 No. 831n approved a single sample of the Medical certificate of admission to driving vehicles and instructions for filling it out.

In this regard, clause 19 of the Procedure contains a rule according to which the Procedure does not apply if the legislation of the Russian Federation establishes a different procedure for issuing a certificate or medical opinion, or another form of certificate or medical opinion.

The procedure establishes the rules for the issuance of two types of documents by medical organizations: certificates and medical reports. In order to obtain one of these documents, you must personally contact a medical organization and present an identity document. Since medical certificates and medical reports can be issued to various categories of citizens located on the territory of Russia, the identity document depends on the status of citizens (clause 2 of the Procedure):

  • for stateless persons permanently residing in the Russian Federation- a document recognized in accordance with an international treaty of the Russian Federation as an identity document of a stateless person;
  • for foreign citizens temporarily residing in the Russian Federation- a passport of a foreign citizen or another document established by federal law or recognized in accordance with an international treaty of the Russian Federation as a document proving the identity of a foreign citizen, with a note on a temporary residence permit in the Russian Federation;
  • for stateless persons temporarily residing in the Russian Federation, - a document recognized in accordance with an international treaty of the Russian Federation as a document proving the identity of a stateless person, with a mark on a temporary residence permit in the Russian Federation, or a document of the established form issued in the Russian Federation to a stateless person who does not have a document certifying his personality.

If you need to get a certificate or medical opinion regarding minors or incapacitated persons, then to obtain it, their legal representatives must contact a medical organization and present a document proving their identity, as well as a document confirming the powers of a legal representative (clause 3 of the Procedure). This is exactly what will be discussed in the article. You will learn how to fill out a pregnancy certificate. We will also consider a sample. In addition, you can find out some of the design features of this document.

When might you need a document?

A medical certificate of pregnancy is often required at the workplace. If the woman's sphere is connected with hard physical labor or there are conditions at work in which it is dangerous for the expectant mother, then the employer may require such a form. In this case, the fairer sex will count on a change in conditions.

If you want to apply to the registry office, and you have a pregnancy certificate, then representatives of the law can speed up the marriage process. On average, future newlyweds have to wait two to four months. However, if you provide the form, you can expect to register within one month. Moreover, the longer the gestation period, the faster everything will happen.

In some cases, airline representatives may ask you for this document. Many companies do not undertake to transport expectant mothers after. In this case, you will need a certificate indicating the period.

There are many other instances that require a specific sample of a pregnancy certificate. In this case, you need to inform your gynecologist that you need a certain form of the document.

To whom and how is the document issued?

Pregnancy certificate (each stamped sample is provided exclusively to expectant mothers. A woman who wishes to receive a form must undergo a thorough examination. examination, and then prescribes control ultrasound diagnostics. This process can take different time: from several hours to one week.

Remember that a pregnancy certificate is valid for a specific period. Usually the document is not issued for more than a month. That is why you should not immediately try to get this form. Only apply for it when you really need it.

Remember that only the expectant mother can get a legal document. None of the women’s representatives can count on such a form, since in order to issue it, it is necessary to undergo a preliminary examination.

Form and sample of pregnancy certificate

If you need this document, then you can contact any medical institution for it. Even private firms issue this form. However, the state certificate of pregnancy (a sample with a seal you will see below) is more credible for several reasons. This form contains the stamp of the institution and the signature of the head physician. The sheet also contains the full name and certificate number of the hospital. If you do not know in which form the document is filled out, you can see a sample pregnancy certificate from your gynecologist. In most cases, a standard form is used.

How to fill out a pregnancy certificate correctly?

When completing this document, you must take into account a sample of medical certificates of pregnancy. Experienced gynecologists and obstetricians already know what to write in this or that column. Also, doctors are provided with a completed form, which can be guided by. To enter your personal information, the doctor will need some documents.

Medical facility data

A pregnancy certificate (a sample with a seal is available in our article) provides for the indication of the name of the institution that issues the form. At the very top of the sheet, the gynecologist writes the name of the clinic. Next, enter the address and phone number. Some certificates are provided for filling with an already prepared header. In this case, the doctor skips this item.

Patient's personal data

After specifying information about the medical institution, the doctor enters information about the patient. In the uppermost column, the surname, name and patronymic are entered. After that, you need to write your date of birth. The place of residence or registration must be indicated.

Many medical institutions, in addition to personal data about the patient, also enter the number with the policy series. Your passport may also be required. However, these are more rare cases.

The fact of establishing pregnancy

After filling in the main fields of the document, the doctor issues his opinion. If a woman is registered for pregnancy, then it is not difficult for a physician to diagnose and give recommendations.

When the fairer sex is in the earliest stages, the physician indicates all laboratory tests that made it possible to establish the presence of a fetus in the womb. Such manipulations include: manual examination, diagnostic ultrasound, blood test, and so on.

It is worth noting that a pregnancy certificate most often does not provide for an indication of the term. However, sometimes this is exactly what the expectant mother needs. If you need to write an estimated due date, do not forget to inform your doctor about it.

Indication of the place of provision

After filling in all the data, the doctor indicates the organization for which this certificate was written. Most often, here you can find the following entry: "To be provided at the place of demand." However, some doctors prefer to indicate exactly where this document should be given.

Doctor and clinic data

At the very bottom, the medic must enter his own data. Here you can get by with the surname and initials. However, you need to write the position of a specialist. Also, a seal is put, which the doctor has for personal use.

After that, the certificate must be certified by the chief specialist of the hospital. For this, there must be a special column. The chief physician indicates his last name and signs. A confirmation seal with a coat of arms is required. The stamp can be triangular or circular.

Alternative filling method

In some cases, your doctor may give you a free certificate form. In this case, the physician does not take into account the samples of this document and enters all the information arbitrarily. Such certificates must include the last name, first name and patronymic of the patient, as well as the year of birth. Below is an immediate conclusion and recommendations. Such a certificate must necessarily have the stamp of a doctor and a medical institution. It does not matter at all that it is written in free form.

Private clinics also do not adhere to a specific template for this document. Gynecologists and specialists issue a certificate with a similar seal of a doctor and a medical institution, as well as all the necessary signatures. However, in this case, you will have to pay a certain amount for the inspection and the provided document. Typically, the average price category for such a service is in the range from 500 to 2000 rubles.

Summing up and a little conclusion

So, now you know what a pregnancy certificate is and why you may need this document. Remember that some clandestine firms offer to purchase such a form for a certain amount of money. However, this number does not work in most cases. Representatives of the law quickly declassify counterfeit forms and severely punish fraudsters and those who used their services.

You can get a pregnancy certificate at any public medical institution or private clinic, provided that you are really preparing to become a mother soon. Fill out the paperwork correctly. I wish you success!
